			<content:encoded><![CDATA[<p>It is an uncomfortable thought to imagine that out of every 3 people alive today, 1 has arthritis of one form or another. While we are grappling with HIV and Aids infections and cancer management, it shocks you even further when you realise that no other illness or accident or catastrophe causes more disability than this disease of the joints. In America, 1.2 % of GDP is spent in managing this disease.</p>
<p>But it is also a known fact that there is no cure for arthritis. What then is the point of research? Is it to simply come up with newer ways of managing it? The answer is yes. But there is a whole branch of researchers who have invested lots of talent in observing the development of this illness with a view to expose an observable pattern on how it begins.</p>
<p>Everyone feels sorry that a person they know and love is afflicted by this disease. But what can they do? You feel helpless that you cannot even prevent the next person from getting it. other than secondary osteoarthritis that is caused from repeated trauma on joints, most other forms of this malady are rather obscure when they are in their early stages.</p>
<p>Professor James Cook, who is with veterinary medicine and surgery, said that according to their findings, there is a pattern in the way canine arthritis begins. It is a matter of certain genes being found around the joints just before the disease is diagnosed. The research is still in its early stages and there are difficult in its progress.</p>
<p>While the researchers working with man&#039;s best friend have identified this gene, they have not yet been able to find a way of either knowing its source or interfering with its work there. It is hoped that once they establish such salient points, it is going to be possible to stem arthritis in people as well.</p>
<p>The problem has traditionally been to diagnose it early. If this research comes to a successful close, we shall be ahead of that goal already. This means that we shall not only diagnose early but pinpoint to specific individuals that they are at risk of getting the disease.</p>
<p>By so doing, many will change their lifestyles and improve their lives. This has already been suggested as a way of managing the illness of the joints. Exercise, diet, pain and inflammation control are current areas of concern with today&#039;s patient. It will soon be only a matter of diet, exercise and specialised pre-emptive surgery and pharmaceutical regimes. But that all depends on the route this research will take and the magnitude of its success. Professor James cook presented his paper at a meeting of the orthopedic Research Society in D.C. in February, 2005.</p>

All around the world, we have such foundations that put mind and dollar to the cause of ensuring the world is a better place for the sufferers of this malady. Among the goals of these foundations is to support research, work with companies that seek to provide pharmaceutical solutions to this disease as well as to spread information on how best the illness can be managed.</p>
<p>The American foundation for the disease is a non-profit making organisation which has over half a million volunteers and close to 200 service points where information support and care for arthritis patients can be procured.</p>
<p>It is among the campaigns&#039; strategies to get people to respond to this illness with greater understanding. For instance, specialised exercise techniques are taught to surfers as well as care givers so as to extend both the meaningful lives of patients as well as slow arthritis&#039; ruinous reign. They hold workshops attended by members as well as other willing participants. Mainly, speakers range from sufferers, doctors and care givers, encouraging each other and bringing to light any new skill, medicine, herb, technique and confirmed therapies.</p>
<p>Diets are also crucial in the word spread by these foundations. As much as pharmaceutical companies are partners with these organisations. Members can easily see a specific leaning towards natural therapies, herbs and non-pharmaceutical cures.</p>

With a click of the mouse, many sufferers and care givers are able to stay abreast with the world of arthritis. <p>It will also assure you to note that about half a billion dollars have been used in the awareness and fight against arthritis by these foundations alone. The governments of the world have their own small outfits with regard to health ministry priorities and they are able to respond to this problem alongside other diseases and pressing issues.</p>

<p>Glucosamine and chondroitin are widely advertised as remedies for osteoarthritis. The benefits, however, are not clear.</p>
<p>A qualitative assessment of various trials were done by T.E. McAlindon et al. to evaluate the benefits of glucosamine and chondroitin products for osteoarthritis of the knee and/or hip.</p>
<p>The studies were randomised, double blind, placebo controlled of four weeks duration. Fifteen studies reached the criteria and were included in the analysis.</p>
<p>Conclusions: Trials of glucosamine and chondroitin products for osteoarthritis of the knee and hip showed moderate to higher effects but there were quality issues and possible bias suggesting that benefits were exaggerated.</p>
<p>They concluded, however, that some degree of benefits appear probable.</p>
<p>A trial by H.Muller-Fassbender et al. in 1994 compared glucosamine sulfate to ibuprofen in osteoarthritis of the knee. The result showed that glucosamine was as effective as ibuprofen but without the potential serious side effects.</p>

<p>There are certain arthritic conditions that we know a specific cause for. Gout, for example, we know is caused by a lack of an enzyme, xanthine oxidase, and as a result the level of uric acid in the blood is higher than normal. This high level results in crystals separating out and being deposited in joints and other areas of the body.</p>
<p>In most cases of arthritis there are more than one factors involved. Some people have a genetic risk that leaves them susceptible to arthritis. Some arthritis runs in families but the precise genetic link is not yet clear.</p>
<p>Environmental factors contribute to arthritis. Individuals involved in repetitive high physical demand in a particular joint will be prone to arthritis. Injury can cause damage or mis-alignment of the joint leading to undue wear.</p>
<p>Infections in the joint (septic arthritis) can lead to arthritis. Some general infections can cause joint pains but this goes away when the infection subsides.</p>
<p>There is no clear evidence that climate contributes to arthritis. These joint problems are worldwide. I know a lady with osteoarthritis. She could tell with reasonable accuracy the weather outside when she wakes up in the morning by the level of stiffness and pain.</p>
<p>Another patient who endures regular winter aches and pain in UK, was remarkably pain free during a winter spent in the Spain where the weather in mild and the sky is blue. Pain is subjective and the intensity does not necessarily depend on the severity of arthritis. Of course, winters in UK are cold, damp and gloomy and the sky usually grey.</p>

Level of satisfaction and success especially at work and home impacts on pain level and much as the personality profile.</p>
<p>A placebo surgery trial on osteoarthritic knee where no corrective surgery occurred produced significant improvement on pain level and functional impairment. Expectation was responsible for the improvement.</p>
<p>Crepitus (grating noise) is present in osteoarthritis. You can hear the noise as the joint is moved. Bony enlargement can occur and often the joint looks mis-shapened. Restricted movement is not uncommon. The joint may feel warm or swollen (effusion) following synovitis. Weakening of supporting muscles and wasting can follow.</p>
<p>Remember osteoarthritis is not only due to <a class="zem_slink" title="Wear and tear" rel="wikipedia" href="http://en.wikipedia.org/wiki/Wear_and_tear">wear and tear</a>. There may be other underlying problems associated wirth arthritis. Conditions such as fibromyalgia, gout and sepsis must be considered when having treatment for osteoarthritis.</p>
<p>If you suspect you have osteoarthritis, make sure you see your family doctor. Accurate assessment, diagnosis and investigation of potential associated illnesses will help both in the short and long term when having osteoarthritis treatment.</p>

<p>Osteoarthritis can be grouped into two types, primary and secondary.<br />
In primary, the cause is generally unknown and in secondary it can develop following damage to the joint or mis-alignment.</p>
<p>When osteoarthritis develops the surface of the cartilage splits, and focal erosions develops leading to exposure of the underlying bone. The exposed bone becomes worn and smooth. This is called eburnation. The synovial fluid can enter the exposed bone and if this happens, cysts (subchondrial cysts) can develop.</p>
<p>Osteophytes develop around the joint margin. The joint becomes unstable as the capsule becomes lax and muscles wasting develops. Effusion and inflammation may be due to trauma or irritation.</p>
<p><strong>Primary Osteoarthritis:</strong> The cause is unknown but factors such as ageing, cartilage wear, obesity, metabolic disturbance, bone wear and genetic factors should be considered. It is most common in the elderly but can develop in young people. Obesity is an important factor. Additional weight will create extra pressure on joints and lead to wear. In primary osteoarthritis the ankle joints are spared.</p>
<p><strong>Secondary Osteoarthritis:</strong> Damage or mal-alignment following trauma can lead to premature wear of joints.</p>
<p><strong>Trauma:</strong> This can be intra-articular fracture and can cause mal-alignment, in the lower limb or a shorter leg. All these can contribute to extra wear. Occupation also contribute to trauma ie footballers are likely to have ankle injuries.</p>
<p><strong>Genetic:</strong> These include bone and cartilage dysplasia, congenital dislocation of the hip, Perthe’s disease, slipped femoral epiphysis, and hypermobility joints.</p>
<p><strong>Inflammation:</strong> Includes septic arthritis, rheumatoid arthritis and gout.</p>

<p>Patients with Rheumatoid Arthritis should benefit from the new range of drugs called Tumour Necrotic Factor Inhibitors (<a class="zem_slink" title="TNF inhibitor" rel="wikipedia" href="http://en.wikipedia.org/wiki/TNF_inhibitor">Anti-TNF</a>).</p>
<p>Tests on three of these new drugs slowed progression and reduced symptoms by up to 50%.</p>
<p>Research at the Medical University of Vienna, Austria, has shown that these three drugs are most effective in combination with the standard treatment, methotrexate.</p>
<p>In their published article, the team said, &#8220;The emerging agents show that a new era has started in the treatment of Rheumatoid Arthritis.&#8221;  UK expert, Paul Emery said, &#8220;They are strikingly effective and they work  on different targets from the existing drugs. That&#8217;s the joy of it.&#8221;</p>
<p>The three drugs in the trial are <a class="zem_slink" title="Rituximab" rel="wikipedia" href="http://en.wikipedia.org/wiki/Rituximab">MabThera</a> (rituximab), Orenca (abatacept), and tocilizumab.</p>
<p>These drugs are, however, not cheap. It is estimated that it will add around £250 million to the NHS drug bill.</p>
<p>Rheumatoid Arthritis is a debilitating joint disease and develops when <a class="zem_slink" title="Immune system" rel="wikipedia" href="http://en.wikipedia.org/wiki/Immune_system">the immune system</a> attacks the joints causing destruction, stiffness and pain. It is estimated that there are about 400,000 rheumatoid arthritis patients in the UK.</p>
<p>Orenca was the first drug to target T cell in blood and tissue. Other Anti-TNF drugs are <a class="zem_slink" title="Etanercept" rel="homepage" href="http://www.enbrel.com/">Enbrel</a> (etanercept) and <a class="zem_slink" title="Infliximab" rel="wikipedia" href="http://en.wikipedia.org/wiki/Infliximab">Remicade</a> (inifliximab).</p>
<p>As these drugs affect the immune system, there is a higher risk of infections. They can cause headaches, fevers and increase blood cholesterol.</p>
<p>Ailsa BOSWORTH of the National Rheumatoid Arthritis Society said, &#8220;It means that we have some choice, and that&#8217;s very important if you are 22 and facing a lifetime of the Disease.&#8221;</p>
